Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Dhea interacts with.

Theoretically, DHEA might increase the levels of drugs metabolized by CYP3A4.
Some preliminary evidence shows that DHEA may inhibit CYP3A4; however, the clinical significance of this potential interaction is not known.

Here's the gist: your body clears bedaquiline using a liver enzyme called CYP3A4. There is some early, lab-based hint that DHEA might slow that enzyme down a bit. If it did, bedaquiline could build up a little more than expected, which in theory could nudge up the risk of its known issues, mainly QT prolongation (a change in heart rhythm) and liver irritation.

I want to be honest, though, this is anecdotal, theoretical stuff, not something proven in real patients, so the practical effect may be small or none. Bedaquiline is a serious TB medicine that needs steady, predictable levels, so please don't add DHEA on your own. Run it by your pharmacist or TB doctor first.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
