Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Dhea interacts with.

Theoretically, DHEA might increase the levels of drugs metabolized by CYP3A4.
Some preliminary evidence shows that DHEA may inhibit CYP3A4; however, the clinical significance of this potential interaction is not known.

Here's the gist for irinotecan (Onivyde): your liver uses an enzyme called CYP3A4 to help process this chemo drug. DHEA may, at least in theory, slow that enzyme down a little. If that happens, irinotecan could clear more slowly and build up in your body, which might mean stronger side effects like severe diarrhea, nausea, mouth sores, fatigue, or a bigger drop in your blood counts (raising infection risk).

That said, this concern rests on weak, mostly lab-based evidence, and we honestly don't know if it matters in real life. Because irinotecan is potent chemo with serious possible effects, please don't add DHEA on your own. Run it by your oncology team or pharmacist first so they can weigh in.

  • Irinotecan lipid complex is used in combination with other medications to treat pancreatic cancer that has spread to other parts of the body that has worsened after treatment with other chemotherapy medications.
