Here's the gist for Koselugo (selumetinib): your child's liver uses an enzyme called CYP3A4 to help clear this medicine. DHEA may slow that enzyme down a bit, at least in early lab-style observations. If selumetinib clears more slowly, more of it could linger in the body, which in theory might mean stronger effects and a higher chance of its usual side effects, things like diarrhea, nausea, rash, mouth sores, tiredness, or muscle aches.

I want to be honest, though: this is theoretical and based on weak, anecdotal evidence, so we don't really know how much it matters in real life. Because selumetinib is a cancer-type drug with careful dosing, please don't start or stop DHEA on your own. Check with your oncology team or pharmacist first.

Here's the honest picture for Koselugo (selumetinib) with DHEA. The interaction we have on file is really about DHEA's tendency to make platelets a little less sticky, which can slightly raise bleeding risk when paired with blood thinners or antiplatelet drugs. Selumetinib itself isn't a classic anticoagulant, but it can lower blood counts and cause its own bleeding-type issues, like nosebleeds, easy bruising, or bleeding gums.

So the worry is mostly additive. This is based on lab and limited human data, so it's theoretical, not proven. Still, if you notice unusual bruising or bleeding, mention it. Please don't start or stop either one on your own. Check with your child's oncologist or pharmacist first, since DHEA is a hormone and deserves a careful conversation.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
