CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Chlorpheniramine Maleate, Dextromethorphan HBr falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist with this combo cold medicine: diosmin might slow down a liver enzyme called CYP3A4, which helps clear the dextromethorphan (the cough suppressant) in your Vicks Formula 44M. If that breakdown slows, a little more dextromethorphan could hang around, which in theory could mean more drowsiness, dizziness, or that foggy, lightheaded feeling. The chlorpheniramine (an antihistamine) already causes sleepiness on its own, so you could feel extra groggy.
I'll be honest, this is mostly theoretical and the lab research is mixed. The clearest signal came from carbamazepine, a different drug, not this product. Acetaminophen isn't really affected here. Occasional cold-medicine use is unlikely to cause trouble, but if you take diosmin daily, just run it by your pharmacist.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

Chlorzoxazone is metabolized by CYP2E1 enzymes. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of chlorzoxazone (Paraflex 250) 250 mg increases blood levels of chlorzoxazone by 34% and decreases chlorzoxazone clearance by 40%. It is speculated that diosmin reduces the metabolism of chlorzoxazone by inhibiting CYP2E1.

Here's the piece that matters in this combo: acetaminophen (the Tylenol part of your cough and cold medicine) is partly handled by a liver enzyme called CYP2E1. Diosmin may gently slow that enzyme down. The catch is that CYP2E1 is the same pathway that turns a small amount of acetaminophen into a byproduct that can stress the liver. So in theory, slowing it could actually be neutral or even slightly protective rather than harmful, but the honest answer is we just don't have good human data on this exact pairing.
This is a theoretical interaction based on lab reasoning, not real-world cases, so I wouldn't lose sleep over it. The bigger everyday thing with this product is simply not doubling up on acetaminophen from other cold or pain products. If you take diosmin regularly, mention it to your pharmacist so they can keep an eye on the whole picture.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies.
