Why this applies to Perampanel

Perampanel falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the basic idea with perampanel (Fycompa) and diosmin. Diosmin may gently slow down a liver enzyme called CYP2C9, which is one of the enzymes that helps your body clear certain medications. The thinking is that if that pathway slows, a drug it handles could build up a bit and cause stronger effects.

For perampanel specifically, I want to be honest: it's mainly broken down by a different enzyme (CYP3A4), so CYP2C9 plays only a minor role. Even so, if levels did creep up, you might notice more of perampanel's usual side effects, like dizziness, sleepiness, trouble walking, or mood changes. This concern is theoretical and based on pharmacology, not real-world reports with perampanel. Still, since seizure meds matter, run it by your pharmacist or doctor before adding diosmin.

Perampanel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist for Perampanel (Fycompa): your liver clears it mainly through an enzyme called CYP3A4. Diosmin may gently slow that same enzyme down. If it does, perampanel could clear a little more slowly and build up in your blood, which can mean stronger effects and more side effects. With perampanel that often shows up as extra drowsiness, dizziness, trouble walking or balancing, and sometimes mood or irritability changes.

I want to be honest about the strength of this, though. The evidence is mostly theoretical and based on lab and animal work, plus one small human study using a different drug (carbamazepine). It may not happen at all. Please don't change anything on your own. Just check with your pharmacist or doctor before adding diosmin so they can keep an eye on you.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
