Palbociclib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ergot interacts with.

Ergotamine, a constituent of ergot, is a substrate of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that inhibit CYP3A4 might increase the risk of ergot toxicity. Some of these drugs include amiodarone (Cordarone), clarithromycin (Biaxin), diltiazem (Cardizem), erythromycin (E-mycin, Erythrocin), indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase), and many others.

Here's how this one works. Palbociclib (Ibrance) is one of those medicines that can gently slow down the CYP3A4 enzyme, which is the same enzyme your body uses to break down ergotamine from ergot. So if you took ergot while on Ibrance, the ergot could clear out of your system more slowly and build up, raising the risk of ergot toxicity. That can mean badly narrowed blood vessels, cold or tingly hands and feet, leg cramping, headache, and in serious cases reduced blood flow to the limbs.

The good news is this is based on weak, theoretical evidence. The bigger issue is that crude ergot is poisonous and shouldn't be used as a home remedy at all. Please check with your oncologist or pharmacist before mixing anything new with your cancer treatment.

What is Ergot?

Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.

About Palbociclib

What Is Palbociclib Used For?

Brand name: Ibrance

Palbociclib (brand name Ibrance) is used to treat adults with hormone receptor (HR)-positive, HER2-negative advanced or metastatic breast cancer. It is always given in combination with other medicines — either an aromatase inhibitor as a first hormonal treatment, or with fulvestrant after the cancer has progressed on prior hormonal therapy. It is also used with inavolisib and fulvestrant for a specific subtype (PIK3CA-mutated, endocrine-resistant) of this cancer confirmed by an FDA-authorized test.

Ibrance is also approved in combination with trastuzumab (with or without pertuzumab) and endocrine therapy for maintenance treatment of adults with HR-positive, HER2-positive locally advanced or metastatic breast cancer, following initial induction treatment.
