Acetaminophen, Ibuprofen falls under HEPATOTOXIC DRUGS — the group Garcinia interacts with.
There have been reports of acute hepatitis with elevated liver enzymes associated with garcinia, when taken alone or in combination with other ingredients. Case reports collected from the Drug Induced Liver Injury Network suggest this risk may be greater in people who carry the HLA B*35:01 allele.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Combogesic (the acetaminophen plus ibuprofen combo) and Garcinia. The concern isn't about Garcinia changing how these two pain relievers work in your body. It's that both Garcinia and acetaminophen can, in rare cases, stress the liver. Stacking two things that lean on the liver could in theory add up to a higher chance of liver trouble.
I'll be honest, this is a theoretical caution based mostly on case reports tied to Garcinia, not strong proof. Acetaminophen at normal doses is well tolerated by most people. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. If you have any liver issues or drink alcohol regularly, please check with your pharmacist or doctor first.

ANTICOAGULANT/ANTIPLATELET DRUGS

HCA inhibits platelet aggregation in vitro. The inhibitory effect seems to be greater in platelets extracted from diabetic subjects than non-diabetic subjects.

Here's the gist for Combogesic, which combines acetaminophen and ibuprofen. The ibuprofen half already nudges your platelets to be a little less sticky, so blood may clot a touch slower. Garcinia's main compound, hydroxycitric acid, has shown in test-tube studies that it can do something similar. Put them together and, in theory, you could see a slightly higher chance of bruising, nosebleeds, or stomach irritation.
I want to be honest though: this is a theoretical concern based on lab work, not real patient reports, so it's likely minor for most folks. Still, if you take Combogesic regularly or notice easy bruising or dark stools, please loop in your pharmacist or doctor before adding Garcinia.
