Garcinia and hydrocortisone, pyrilamine, chlorpheniramine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to hydrocortisone, pyrilamine, chlorpheniramine

hydrocortisone, pyrilamine, chlorpheniramine falls under SEROTONERGIC DRUGS — the group Garcinia interacts with.

Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product (a topical mix of hydrocortisone, pyrilamine, and chlorpheniramine) is mostly used on the skin to calm itching and irritation. The concern on file is that Garcinia could theoretically add to serotonergic effects when paired with serotonin-active drugs. Honestly, that link is a weak fit here. The two antihistamines in this product have only mild serotonin activity, and when you apply this as a cream or lotion, very little gets into your bloodstream.

So for a skin product like this, the real-world risk of something like serotonin syndrome is very low. The evidence behind this warning is anecdotal and based on antidepressants, not topical antihistamines. Still, if you ever take chlorpheniramine by mouth alongside Garcinia, or you have liver concerns, check with your pharmacist first.
