Palonosetron falls under SEROTONERGIC DRUGS — the group Garcinia interacts with.

Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.

Here's the gist with Garcinia and your anti-nausea medicine palonosetron (Aloxi). Palonosetron works on serotonin pathways, and Garcinia has been linked, in rare anecdotal reports, to raising serotonin activity in the body. The worry is that stacking two things that nudge serotonin upward could, in theory, increase the chance of a reaction called serotonin syndrome.

To be honest, the evidence here is thin. The reports involved Garcinia with antidepressants, not palonosetron, and palonosetron is usually a short course given around chemotherapy or surgery. Still, signs to watch for would be agitation, a fast heartbeat, sweating, shivering, muscle twitching, or confusion. This isn't something to panic over, but please mention any supplements to your oncology team or pharmacist so they can keep an eye on things.

About Palonosetron Injection

What Is Palonosetron Injection Used For?

Brand name: Aloxi

Palonosetron hydrochloride injection is used in adults to prevent nausea and vomiting caused by moderately or highly emetogenic (nausea-causing) cancer chemotherapy — including both the immediate reaction and the delayed reaction that can happen days later. It is also approved in adults to prevent postoperative nausea and vomiting (PONV) for up to 24 hours after surgery, given right before anesthesia is started.

In children aged 1 month to less than 17 years, it is used to prevent acute (immediate) nausea and vomiting from emetogenic cancer chemotherapy, including highly emetogenic regimens. Posfrea is the brand name under which this intravenous injection is available.
