Why this applies to Amitriptyline

Amitriptyline falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Ginkgo interacts with.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP2C19.
Some clinical research shows that a specific ginkgo leaf extract (Remembrance, Herbs Product LTD) 140 mg twice daily can induce CYP2C19 enzymes and potentially decrease levels of drugs metabolized by these enzymes. However, other clinical research shows that taking ginkgo 120 mg twice daily for 12 days has no effect on levels of drugs metabolized by CYP2C19.

Here's the short version on combining ginkgo with amitriptyline (Elavil). Your liver uses an enzyme called CYP2C19 to help break this antidepressant down. Some studies suggest a specific ginkgo extract might speed that enzyme up a bit, which could lower amitriptyline levels and make it work less well. In real life that might look like your depression symptoms creeping back. Amitriptyline isn't a prodrug, so faster clearance means less effect, not more.

That said, the evidence here is genuinely shaky. One ginkgo product nudged the enzyme, while another regular dose did nothing at all, so this may not happen to you. I wouldn't panic, but please loop in your pharmacist or doctor before adding ginkgo so they can watch how you're doing.

Theoretically, ginkgo might increase levels of drugs metabolized by CYP1A2.
Laboratory research suggests that ginkgo leaf extract can mildly inhibit CYP1A2 enzymes. However, clinical research suggests ginkgo might not affect CYP1A2. Until more is known, use ginkgo cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist: your body uses an enzyme called CYP1A2 to help clear amitriptyline. Lab studies hint that ginkgo might mildly slow that enzyme down. If that happened, amitriptyline could build up a bit and you might notice more of its usual side effects, things like drowsiness, dry mouth, constipation, blurry vision, or feeling lightheaded when you stand up.

The good news is the evidence here is shaky. Test-tube work suggests a small effect, but actual human studies suggest ginkgo probably doesn't change CYP1A2 much. So this is more of a theoretical caution than a sure thing. Still, it's worth a quick word with your pharmacist or doctor before adding ginkgo, and never adjust your amitriptyline on your own.

Theoretically, ginkgo might increase levels of drugs metabolized by CYP2C9.
In vitro, a specific standardized extract of ginkgo leaf (EGb 761) inhibits CYP2C9 activity . The terpenoid (ginkgolides) and flavonoid (quercetin, kaempferol, etc.) constituents seem to be responsible for this effect. Most ginkgo extracts contain some amount of these constituents. Therefore, other ginkgo leaf extracts might also inhibit the CYP2C9 enzyme. However, clinical research suggests that ginkgo might not have a significant effect on CYP2C9 in humans. Ginkgo does not seem to significantly affect the pharmacokinetics of CYP2C9 substrates diclofenac or tolbutamide.

Here's the basic idea: ginkgo may slow down a liver enzyme called CYP2C9, which is one of the pathways your body uses to clear amitriptyline. In theory, if that pathway slows, more of the drug could linger in your system. That could mean stronger effects like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.

That said, I want to keep this in perspective. This concern comes mainly from lab studies, not real people, and human research so far hasn't shown ginkgo meaningfully changing CYP2C9 in practice. So the real-world risk is probably low. Still, ginkgo can also thin the blood a bit, so it's worth a quick chat with your pharmacist or doctor before combining them.

Theoretically, ginkgo might decrease levels of drugs metabolized by CYP3A4.
There is conflicting evidence about whether ginkgo induces or inhibits CYP3A4. Ginkgo does not appear to affect hepatic CYP3A4. However, it is not known if ginkgo affects intestinal CYP3A4. Preliminary clinical research suggests that taking ginkgo does not significantly affect levels of donepezil, lopinavir, or ritonavir, which are all CYP3A4 substrates. Other clinical research also suggests ginkgo does not significantly affect CYP3A4 activity. However, there are two case reports of decreased efavirenz concentrations and increased viral load in patients taking ginkgo. It is suspected that terpenoids from the ginkgo extract reduced drug levels by inducing cytochrome P450 3A4 (CYP3A4).

Here's the short version: amitriptyline is partly handled by a liver enzyme called CYP3A4, and ginkgo might nudge that enzyme to work a little faster. If that happens, your body could clear amitriptyline slightly quicker, which in theory might leave a touch less in your system and make it work less well for your mood or sleep.

I want to be honest about how thin this evidence is. The data on ginkgo and CYP3A4 actually conflict, and amitriptyline is not a prodrug, so there's no activation step to worry about here. Most studies suggest ginkgo doesn't meaningfully shift these drug levels. There's also a separate point worth raising: ginkgo can thin the blood. Before you pair them, just check in with your pharmacist or doctor.
