Why this applies to Aspirin, Caffeine

Aspirin, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the practical side of mixing goldenseal with a product like Anacin (aspirin plus caffeine). Goldenseal tends to block a liver and gut enzyme called CYP3A4. The caffeine part of this combo is mostly cleared by a different enzyme (CYP1A2), so the bigger concern isn't a huge spike, but goldenseal can still slow down how some substances are broken down, which may leave a little more caffeine hanging around than usual. That can mean feeling jittery, a racing heart, trouble sleeping, or an upset stomach.

The evidence here is moderate and based on smaller studies, so this isn't a dramatic, guaranteed problem. Concentrated goldenseal supplements matter more than a sip of any herbal tea. Still, it's worth running this combination by your pharmacist or doctor before pairing them regularly.

Theoretically, goldenseal might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Goldenseal contains berberine. In vitro and animal research shows that berberine can inhibit platelet aggregation. However, this effect has not been reported in humans.

The product you're asking about (sold as Anacin) combines aspirin with caffeine. The piece that matters here is the aspirin, which already thins your blood a bit by keeping platelets from clumping together. Goldenseal contains berberine, and in lab dishes and animal studies that compound seems to do something similar, nudging platelets to stick together less.

So on paper, pairing the two could add up to a slightly higher chance of bleeding or bruising. I want to be honest though: this has only been seen in test tubes and animals, not in actual people, so the concern is theoretical, not proven. Still, if you take aspirin regularly or notice easy bruising, nosebleeds, or bleeding gums, it's worth a quick chat with your pharmacist or doctor before adding goldenseal.
