Efavirenz falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for your HIV medicine. Goldenseal can slow down CYP3A4, one of the liver and gut enzymes that helps clear efavirenz from your body. When that enzyme is blocked, efavirenz isn't a prodrug (it's already active), so the worry is that its levels could creep up rather than drop. Higher levels could mean more of efavirenz's known side effects, like dizziness, vivid or strange dreams, trouble sleeping, mood changes, or feeling foggy during the day.

That said, this is a possible interaction based on modest evidence, and not everyone will notice anything. Because keeping HIV medicines steady really matters, please don't start or stop goldenseal on your own. Check with your pharmacist or HIV provider first so they can help you decide.

Efavirenz is used to treat HIV-1 (human immunodeficiency virus type 1) infection. It is always used together with other antiretroviral medicines — never on its own.

It is approved for adults and for children who are at least 3 months old and weigh at least 3.5 kg (about 7.7 lbs). Both the capsule and tablet forms are approved for this use.
