Why this applies to Vincristine

Vincristine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the situation with these two. Vincristine is cleared from your body mainly by a liver enzyme called CYP3A4. Goldenseal tends to slow that enzyme down, so vincristine can clear more slowly and build up higher than expected in your bloodstream.

Why that matters: vincristine is a chemo drug with a narrow safety window, and its most well-known side effect is nerve damage (think tingling, numbness, weakness, or constipation). Higher-than-planned levels could make those nerve problems and low blood counts more likely. The good news is this is a possible interaction, not a certain one, and your oncology team carefully doses chemo. Please don't start, stop, or change anything on your own. Just tell your oncologist or pharmacist you're considering goldenseal so they can weigh in first.

P-GLYCOPROTEIN SUBSTRATES

Vincristine also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, goldenseal might increase or decrease serum levels of P-glycoprotein (P-gp) substrates.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.

Here's the gist: vincristine is moved out of your cells by a transporter called P-glycoprotein (P-gp). Goldenseal, through its berberine content, can nudge this transporter. The lab evidence is honestly mixed. Some studies say berberine mildly slows P-gp, others say it speeds it up. In real people, goldenseal only bumped levels of another P-gp drug (digoxin) by a small amount, so it doesn't look like a powerful blocker.

Why care at all? Vincristine is a strong chemo drug given by IV, and if levels drift up, its known side effects (nerve tingling and numbness, constipation, muscle weakness) could get worse. This is mostly theoretical, but with chemo I never gamble. Please run goldenseal by your oncology team before using it.

Vincristine Sulfate Injection is approved to treat acute leukemia. It is also used as part of combination chemotherapy regimens for Hodgkin's disease, non-Hodgkin's malignant lymphomas, rhabdomyosarcoma (a soft-tissue cancer), neuroblastoma (a cancer of nerve tissue), and Wilms' tumor (a kidney cancer in children).

Because it works best alongside other cancer medicines, vincristine is almost always given as part of a multi-drug treatment plan rather than on its own.
