Aspirin, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist with goldthread and your Fiorinal (aspirin, butalbital, and caffeine). The berberine in goldthread may, at least in lab settings, slow down a liver enzyme called CYP3A4. That same enzyme helps clear the butalbital and caffeine pieces of this medicine. If it's blocked, those parts could linger a bit longer in your body, which in theory might add to drowsiness, grogginess, or feeling jittery from the caffeine.

The important caveat: this is based only on early lab work, not real reports in people, so I wouldn't lose sleep over it. Still, since this combo already affects how alert you feel, it's worth a quick chat with your pharmacist or doctor before pairing it with concentrated goldthread or berberine supplements.

What Is Aspirin, Butalbital, and Caffeine Used For?

Butalbital, aspirin, and caffeine capsules are used to relieve the symptom complex of tension headache — also called muscle contraction headache. This includes headache pain, psychic tension, and muscle tightness in the head, neck, and shoulders.

This medicine is intended for short-term use. Evidence supporting its safety and effectiveness for repeated or long-term use in recurrent headaches is not available, and extended use is not recommended because butalbital can be habit-forming.
