Losartan Potassium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's a wrinkle worth knowing about losartan. It's actually a prodrug, meaning your body has to convert it into its more active form (called E-3174) for the strongest blood pressure benefit. The enzyme CYP3A4 helps do part of that activation. The berberine in goldthread may gently block CYP3A4 in lab studies, so in theory it could slow that conversion and leave you with a bit less of the active drug working.

If that happened, losartan might not lower your blood pressure quite as well. That said, this is purely theoretical right now, based on test-tube science, and hasn't been seen in real people. Losartan also uses a second enzyme, so the effect may be small. Still, if you take goldthread, mention it to your pharmacist or doctor and keep an eye on your blood pressure readings.

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
