Tisotumab Vedotin-Tftv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist with Tivdak (tisotumab vedotin) and goldthread. Goldthread's main compound, berberine, may gently block a liver enzyme called CYP3A4. The active cancer-killing piece of Tivdak (a payload called MMAE) is cleared partly through that same enzyme. In theory, if you slow that pathway, a bit more of the drug could linger in your body, which might nudge up side effects like fatigue, nausea, nerve tingling or numbness in your hands and feet (neuropathy), bleeding, or eye irritation.

I want to be honest though: this is theoretical, based on lab clues, and it hasn't actually been reported in people. Since Tivdak is a serious chemotherapy infusion, please don't add goldthread or berberine without first running it by your oncology team or pharmacist.
