Why this applies to Cisapride

Cisapride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Here's the gist with hibiscus and cisapride. Your liver uses an enzyme called CYP3A4 to break cisapride down and clear it out. In lab (test tube) studies, hibiscus extract slowed that enzyme a bit. If that happened in your body, cisapride could build up to higher levels than expected.

Why does that matter? Cisapride is one of those drugs that can affect the heart's rhythm (a problem called QT prolongation) when levels run too high, which is exactly why it's so restricted these days. That said, this is purely theoretical and has never been reported in people, and a cup of tart hibiscus tea is unlikely to be an issue. Still, with a drug this sensitive, please run it by your pharmacist or doctor first.
