In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2D6. This interaction has not been reported in humans.

Here's the simple version. The cough suppressant part of this combo, dextromethorphan, is cleared from your body by a liver enzyme called CYP2D6. In lab (test tube) studies, hibiscus extract appears to slow that same enzyme down. If that happened in a person, dextromethorphan could build up a bit and you might feel more of its effects, like drowsiness, dizziness, or a foggy, lightheaded feeling. The guaifenesin part isn't really affected here.

The reassuring news: this is only a theoretical concern based on lab work, and it has never actually been reported in real patients. A cup of tart hibiscus tea is very unlikely to cause trouble. Still, if you're taking concentrated hibiscus supplements, it's worth a quick chat with your pharmacist or doctor.

Here's the simple version: your cough and chest medicine pairs dextromethorphan (the cough suppressant) with guaifenesin (which loosens mucus). Dextromethorphan is partly cleared by a liver enzyme called CYP3A4. In lab dish studies, hibiscus extract has slowed that same enzyme down, so in theory it could let a bit more dextromethorphan linger in your body. That might mean stronger effects like drowsiness, dizziness, or mild stomach upset.
