Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP1A2 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP1A2. This interaction has not been reported in humans.

Here's the gist for Esbriet (pirfenidone). Your liver uses an enzyme called CYP1A2 to clear this medicine from your body. In a test tube, hibiscus extract slowed that same enzyme down. If that happened in real life, pirfenidone could build up a bit and you might notice more of its usual side effects, like nausea, upset stomach, dizziness, tiredness, or skin that's extra sensitive to sunlight.

The honest part: this is only a theoretical concern from lab research, and it has never been reported in actual people. A cup or two of hibiscus tea is unlikely to cause trouble. Still, since pirfenidone is sensitive to this enzyme, I'd run a concentrated hibiscus supplement past your pharmacist or doctor first.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
