Theoretically, taking Hibiscus sabdariffa with antihypertensive drugs might increase the risk of hypotension.
Most clinical evidence suggests that taking Hibiscus sabdariffa reduces systolic and diastolic blood pressure. In animal research, Hibiscus sabdariffa increased the hypotensive effects of single doses of losartan and amlodipine.

Here's the thing to know about pairing hibiscus tea or supplements with treprostinil (Orenitram). Treprostinil is a prostacyclin medicine that widens blood vessels to treat pulmonary arterial hypertension, and one of its built-in effects is lowering blood pressure. Hibiscus has its own mild blood-pressure-lowering action, so stacking the two together could, in theory, push your pressure down a bit further than expected.

If that happened, you might feel some of treprostinil's familiar effects more strongly: dizziness, lightheadedness, headache, or flushing, especially when standing up quickly. To be clear, this concern is theoretical, based on how these things work rather than firm studies in people. Occasional hibiscus tea is unlikely to be a big deal, but before using concentrated supplements, check with your pharmacist or doctor.

Treprostinil also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.

Theoretically, Hibiscus sabdariffa might reduce the metabolism of CYP2C8 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2C8. This interaction has not been reported in humans.

Here's the gist for your oral treprostinil (Orenitram). Your body clears this medicine in part through a liver enzyme called CYP2C8. In a lab dish, hibiscus extract slowed that enzyme down. If the same thing happened in people, treprostinil could be cleared a bit more slowly and build up, which might nudge up its usual side effects like headache, flushing, jaw pain, nausea, diarrhea, or lightheadedness.

The honest part: this is only a test-tube finding, it has never been seen in real patients, and it's rated minor. A cup or two of hibiscus tea is unlikely to be a problem, but concentrated hibiscus supplements are worth a quick check. Please run it by your pharmacist or doctor before combining them.

Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure. Evidence for other uses is limited, and while hibiscus tea is generally well tolerated in food amounts, concentrated supplements should be used cautiously and avoided in pregnancy. About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
