Loperamide falls under ANTICHOLINERGIC DRUGS — the group Iboga interacts with.

Theoretically, iboga might decrease the effectiveness of anticholinergic drugs.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.

Here's the gist for Loperamide. The concern on file is mostly theoretical and comes from lab studies, not human data. The idea is that Iboga may raise levels of acetylcholine (a nerve messenger) by blocking the enzyme that breaks it down. Since Loperamide works in a way that quiets gut activity, more acetylcholine could, in theory, push your gut the other direction and make Loperamide work a little less well, meaning your diarrhea might not settle as nicely.

That said, the evidence is weak, and interestingly the main active compound in Iboga (ibogaine) didn't show this effect in testing. I'll be honest though: Iboga itself carries serious heart rhythm risks and is illegal in the U.S., so my bigger worry is the Iboga, not this interaction. Please talk with your pharmacist or doctor before combining anything.

What Is Loperamide Used For?

Brand names: Imodium, Loperamide

Loperamide controls the symptoms of diarrhea, including traveler's diarrhea, in adults and children 2 years and older. It is available as tablets, capsules, oral solution, and oral suspension across many brand names.

Loperamide Hydrochloride and Loperamide HCl capsules are also approved for chronic diarrhea in adults associated with inflammatory bowel disease, and for reducing the volume of discharge from an ileostomy (a surgical opening in the abdomen that drains waste).
