Pimozide falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with these two together: Pimozide is well known for stretching out the heart's electrical rhythm, what doctors call QT prolongation. In fact, that risk is one reason Pimozide is reserved for severe Tourette's tics when other options haven't worked. Iboga (through its compound ibogaine) has also been tied to QT prolongation in a number of case reports. Stack the two, and you may add one QT-stretching effect on top of another.

Why does that matter? A badly prolonged QT can trigger a dangerous, fast heart rhythm that may cause fainting, palpitations, or worse. This warning leans on anecdotal reports rather than strong studies, but with a drug as cardiac-sensitive as Pimozide, I wouldn't take chances. Please talk with your doctor or pharmacist before combining them.

Theoretically, iboga might decrease the effectiveness of anticholinergic drugs.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.

Here's the thing to know about combining Iboga with pimozide (Orap). The concern on file is mostly theoretical and based on lab dish studies, not real people. In those studies, iboga slowed down an enzyme that breaks down acetylcholine, a brain messenger. That could raise acetylcholine levels and, in theory, push back against pimozide's calming effect on certain nerve signals. Interestingly, the main active piece of iboga, ibogaine itself, did not seem to do this, so the evidence is shaky.

Two bigger points matter here. First, Iboga is illegal in the U.S. and can trigger dangerous heart rhythm problems, and pimozide is already a drug that affects heart rhythm, so stacking them is genuinely risky. Second, please don't change anything on your own. Talk with your pharmacist or doctor first.

We do not recommend its use outside of tightly controlled research settings.

What Is Pimozide Used For?

Brand name: Orap

Pimozide tablets are used to suppress motor tics (involuntary movements) and phonic tics (involuntary sounds or words) in people with Tourette's Disorder. It is reserved for patients whose daily life or development is severely affected by their tics — it is not intended as a first-line treatment, and it is not used for tics that are mild or mainly cosmetic concerns.

Pimozide is not meant for tics caused by other conditions or medications, and it should not be used for simple tics unrelated to Tourette's Disorder.
