Here's the short version: tenecteplase (TNKase) is a powerful clot-buster given in the hospital during emergencies like a heart attack. Its whole job is to break down clots, so bleeding is already its biggest risk. Inositol nicotinate, a 'no-flush' niacin product, is thought to have mild clot-dissolving (fibrinolytic) effects of its own. Stacking the two together could, in theory, nudge that bleeding risk a bit higher.

I want to be honest, though: this concern is theoretical, based on how the supplement behaves rather than real cases with tenecteplase. And since TNKase is only used in critical, closely watched settings, your medical team controls everything around it. Still, always tell your doctor and pharmacist about any supplements you take so nothing gets overlooked.
