Khat and Dexmethylphenidate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

STIMULANT DRUGS

Why this applies to Dexmethylphenidate

Dexmethylphenidate falls under STIMULANT DRUGS — the group Khat interacts with.

Cathinone, a constituent of khat, appears to have sympathomimetic effects and can increase blood pressure and heart rate. Theoretically, combining khat with other central nervous system (CNS) stimulant drugs might increase the risk of cardiovascular toxicity. Some CNS stimulants include nicotine, cocaine, sympathomimetic amines, and amphetamines.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing with Focalin (dexmethylphenidate): it's already a stimulant that nudges up your heart rate and blood pressure. Khat works through cathinone, a chemical that acts a lot like amphetamine and stimulates your system in a similar way. Stack the two together and you're essentially pressing the same gas pedal twice, which could push your heart and blood pressure higher than either alone.

What that might look like is a racing or pounding heartbeat, jitteriness, a spike in blood pressure, trouble sleeping, or feeling anxious or wired. I'll be honest, this concern is theoretical, based on how both substances behave rather than hard studies. Still, khat is illegal in the U.S. and risky on its own, so please don't combine them and talk with your doctor or pharmacist with any questions.

From our pharmacist’s overview on the Dexmethylphenidate page

“Dexmethylphenidate (Focalin) is itself a prescription stimulant for ADHD, which is exactly why this list deserves real care rather than a quick skim. More than half of the 85 ingredients on file stimulate the nervous system too, so combining them can compound the racing heart, raised blood pressure, and sleeplessness the drug can alrea…”
