Here's the concern with pairing melatonin and Elrexfio (elranatamab). Elranatamab is a bispecific antibody used for multiple myeloma, and it works partly by revving up your immune system's T-cells to attack cancer cells. Melatonin, on the other hand, is thought to gently stimulate immune activity too. So this isn't a case of one drug being broken down differently, it's about the two possibly pushing on the immune system in ways we don't fully understand together.

Honestly, this interaction is theoretical and based on weak evidence, so I don't want to alarm you. But because elranatamab carries real risks like cytokine release syndrome and serious infections, I'd rather you loop in your oncology team or pharmacist before adding melatonin, just to be safe.
