Azilsartan, Chlorthalidone falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.

Here's the gist with Edarbyclor (azilsartan plus chlorthalidone) and neem. Azilsartan is partly handled by a liver enzyme called CYP2C9. In test-tube studies only, neem leaf extract seems to slow that enzyme down. If that held true in people, your body might clear azilsartan a little more slowly, so its blood-pressure-lowering effect could be a touch stronger than expected.

In plain terms, that could mean more dizziness, lightheadedness when standing, or unusually low blood pressure. But I want to be honest: this is theoretical, based on lab data, and hasn't been seen in real patients. I wouldn't lose sleep over it, but if you take neem regularly, mention it to your pharmacist or doctor and keep an eye on your blood pressure readings.
