Neem + Cimetidine
Cimetidine brand names: Tagamet HB, Tagamet, Cimetidine Injection.
Neem has a moderate interaction with Cimetidine. Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.

Here is the interaction we found between these two, with the evidence behind it.
P-GLYCOPROTEIN SUBSTRATES
Why this applies to Cimetidine
Cimetidine falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body uses a tiny pump called P-glycoprotein to help move cimetidine out through your kidneys. Lab studies (done in test tubes, not people) suggest neem leaf extract may slow that pump down. If that happened in real life, a bit more cimetidine could hang around in your body, which might mean slightly stronger effects or side effects like headache, dizziness, or stomach upset.
I want to be honest about how soft this evidence is. We've only seen it in lab dishes, never reported in actual patients, so for most people this is more theoretical than alarming. Still, if you're planning to use concentrated neem supplements (not just toothpaste) alongside cimetidine, it's worth a quick chat with your pharmacist or doctor first.

About Neem
What is Neem?
- Skin conditions
- Dental and gum health
- Antimicrobial and antiparasitic
- Digestive upset
- Blood sugar support
- Insect repellent/pesticide
Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.
About Cimetidine
What Is Cimetidine Used For?
Brand names: Tagamet HB, Tagamet, Cimetidine Injection
Cimetidine tablets sold over the counter — including brands like Acid Controller, Acid Reducer, Basic Care Acid Reducer, Equate Cimetidine, and cimetidine 200 — are used to relieve heartburn associated with acid indigestion and sour stomach, and to prevent heartburn brought on by eating or drinking certain foods and beverages.
Prescription-strength cimetidine tablets and Cimetidine Hydrochloride Oral Solution are used for more serious conditions: short-term treatment of active duodenal ulcers (ulcers in the upper small intestine) and benign gastric ulcers (stomach ulcers), maintenance therapy to keep duodenal ulcers from coming back, healing of erosive GERD (acid reflux that has damaged the lining of the esophagus), and treatment of conditions that cause the stomach to make too much acid — such as Zollinger-Ellison Syndrome, systemic mastocytosis, and multiple endocrine adenomas.

Cimetidine Is Linked to Lower Levels of 7 Nutrients
Natural Medicines records Cimetidine as a medicine that can lower these. This is not an interaction with Neem — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
