Selexipag falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.

Here's the situation with neem and your Uptravi (selexipag). Your body relies on an enzyme called CYP2C8 to help clear selexipag and its active form. Lab studies suggest that neem leaf extract may slow that enzyme down. If that happened in a real person, more of the drug could linger in your system, which might turn up its effects and side effects.

For selexipag, that could mean more headache, jaw pain, flushing, nausea, diarrhea, or muscle aches. That said, this is only a theoretical concern seen in test tubes, not something reported in actual patients. Still, since PAH is serious and the dose matters, I'd check with your pharmacist or doctor before adding concentrated neem supplements.

What Is Selexipag Used For?

Brand name: Uptravi

Selexipag tablets are used to treat pulmonary arterial hypertension (PAH, WHO Group I) — a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to reduce the chances of being hospitalized because of PAH.

Uptravi (selexipag) tablets are approved for both adults and children aged 2 years and older. Uptravi for injection (intravenous) is also available for adult patients who are already on Uptravi tablets but temporarily cannot take medicine by mouth.
