Sodium Nitroprusside falls under ANTIHYPERTENSIVE DRUGS — the group Nicotinamide Riboside interacts with.

Theoretically, nicotinamide riboside may have additive effects with antihypertensive drugs, potentially increasing the risk for hypotension.
One small clinical study suggests that nicotinamide riboside can modestly reduce blood pressure in hypertensive patients. However, other clinical research did not find a reduction in blood pressure when compared with control.

Here's the practical side of this one. Sodium nitroprusside is a powerful blood pressure lowering drug given only through an IV in a hospital, usually for blood pressure emergencies. Nurses and doctors are watching your numbers very closely and adjusting the drip minute by minute. The theory here is that nicotinamide riboside (a form of vitamin B3) might nudge blood pressure down a little on its own, which could in theory add to the drug's effect and lead to too much of a drop, meaning dizziness, a fast heartbeat, or fainting.

Honestly, this concern is mostly theoretical, and the human evidence for NR lowering blood pressure is mixed and weak. Since nitroprusside is hospital-controlled, the team can simply adjust the dose. Still, mention any supplements you take to your care team so they have the full picture.
