Noni and Candesartan Cilexetil, Hydrochlorothiazide

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Noni interacts with whole categories of medication. Candesartan Cilexetil, Hydrochlorothiazide falls into 2 of those categories, so it can interact with Noni in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Candesartan Cilexetil, Hydrochlorothiazide

Candesartan Cilexetil, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Noni interacts with.

Theoretically, noni may increase the risk of hypotension when used in combination with antihypertensive drugs.
Preliminary clinical research suggests that drinking noni juice can reduce blood pressure in individuals with hypertension.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your Atacand HCT already lowers blood pressure two ways, with candesartan relaxing your blood vessels and hydrochlorothiazide helping your body shed extra fluid and salt. Some early research suggests noni juice may nudge blood pressure down a bit on its own, so pairing the two could push your numbers lower than you want.

If that happens, you might feel dizzy or lightheaded, especially when standing up quickly, or feel unusually tired or weak. This concern is mostly theoretical and based on weak, anecdotal reports, so it's not something to panic over. Still, if you'd like to try noni, mention it to your pharmacist or doctor first, and keep an eye on your blood pressure readings at home.

ANGIOTENSIN RECEPTOR BLOCKERS (ARBs)

2 of 2

Candesartan Cilexetil, Hydrochlorothiazide also falls under ANGIOTENSIN RECEPTOR BLOCKERS (ARBs).

Theoretically, combining noni and ARBs might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ARBs, which can also increase potassium levels.

Here's the gist with Atacand HCT (candesartan plus hydrochlorothiazide) and noni. The candesartan part is an ARB, and ARBs tend to nudge your potassium levels upward. Noni juice happens to carry a fair amount of potassium itself, roughly 6 mEq per 100 mL. So the worry is that drinking a lot of noni alongside this medicine could push your potassium too high, a condition called hyperkalemia.

The good news is the water pill (hydrochlorothiazide) in your combo actually tends to lower potassium, so it partly offsets things. And honestly, this concern is based on weak, theoretical evidence. Still, high potassium can cause muscle weakness, tingling, or an irregular heartbeat. If you have kidney trouble or want to use noni regularly, please check with your pharmacist or doctor first.

Noni is a tropical fruit used widely in folk medicine and sold mostly as a juice or supplement, but solid human evidence for its many claimed benefits is limited. While moderate juice intake is generally well tolerated, there are rare reports of liver problems, and it should be avoided in pregnancy and breastfeeding. Talk with your pharmacist or doctor before using it, especially if you have kidney or liver disease.
