Here's the simple version: Entresto (sacubitril/valsartan) already lowers your blood pressure as part of how it helps your heart. Noni juice, based on some early research, may also nudge blood pressure down a bit. Put the two together and the worry is that your pressure could drop a little too far.

If that happens, you might notice dizziness, lightheadedness (especially when standing up), tiredness, or feeling faint. To be clear, this is a theoretical concern backed mostly by anecdotal reports, not strong studies, so it's not a reason to panic. Still, since Entresto is a serious heart medicine, it's smart to mention any noni use to your pharmacist or doctor and keep an eye on how you feel.

Here's the thing to know about Entresto (sacubitril/valsartan): the valsartan part can nudge your potassium levels up on its own. Noni juice is naturally rich in potassium (roughly 6 mEq per 100 mL), so drinking a lot of it alongside Entresto could push your potassium higher than you'd want. That's called hyperkalemia, and when it gets significant it can cause muscle weakness, a slow or irregular heartbeat, or in serious cases dangerous heart rhythm problems.

I'll be honest, this concern is mostly theoretical and based on the potassium content plus anecdotal reports, not strong studies. An occasional small glass likely isn't a big deal for most folks, but regular or concentrated noni supplements are where I'd be cautious. Please check with your pharmacist or doctor before mixing them, especially since they may already be watching your potassium.

Here's the simple version. Entresto contains valsartan, which is part of a group of blood pressure medicines that tend to hold onto potassium in your body. Noni juice is naturally rich in potassium (roughly 6 mEq in every 100 mL). So when you drink a lot of noni alongside Entresto, those two effects can stack and push your potassium higher than it should be.

Noni is a tropical fruit used widely in folk medicine and sold mostly as a juice or supplement, but solid human evidence for its many claimed benefits is limited. What Is Valsartan and Sacubitril Used For?

Brand name: Entresto

Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).

It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
