Here's the gist: noni juice may, on its own, nudge your blood pressure down a little. Telmisartan (Micardis) is already doing that job by relaxing your blood vessels. Stack the two together and you could end up lower than you bargained for. That can show up as dizziness, lightheadedness, feeling tired, or even a faint feeling when you stand up too fast.

I'll be straight with you, though: this is based on anecdotal evidence and theory, not strong studies, so for most folks it's a mild, manageable concern rather than a danger. An occasional culinary amount is unlikely to matter much, but concentrated daily noni juice is worth a chat with your pharmacist or doctor first, especially if your pressure already runs low or you have liver or kidney issues.

Theoretically, combining noni and ARBs might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ARBs, which can also increase potassium levels.

Here's the thing to know about pairing noni with telmisartan. Telmisartan is a blood pressure medicine that, as part of how it works, tends to nudge your potassium levels up a little. Noni juice is naturally rich in potassium (roughly 6 mEq per 100 mL). So drinking a lot of noni alongside telmisartan could stack potassium on top of potassium, and that may raise your risk of hyperkalemia (too much potassium in the blood).

Honestly, this is more of a theoretical concern based on noni's potassium content, not strong study data. High potassium can cause muscle weakness, tingling, or in rare cases an irregular heartbeat. The bigger risk is if you have kidney trouble or take other potassium-raising meds. An occasional small glass is unlikely to be a problem, but please check with your pharmacist or doctor before using noni regularly.

Noni is a tropical fruit used widely in folk medicine and sold mostly as a juice or supplement, but solid human evidence for its many claimed benefits is limited. What Is Telmisartan Used For?

Brand name: Micardis

Telmisartan tablets — sold as Micardis and the generic telmisartan — treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of strokes and heart attacks.

Micardis and telmisartan tablets are also approved to reduce the risk of heart attack, stroke, or death from cardiovascular causes in patients 55 or older who are at high cardiovascular risk and cannot take ACE inhibitors. This use is for patients with a history of conditions such as coronary artery disease, peripheral arterial disease, stroke, TIA (mini-stroke), or high-risk diabetes with signs of organ damage.
