Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Your body relies on a liver enzyme called CYP3A4 to help clear apalutamide (Erleada). The lab note we have suggests that concentrated Padang cassia cinnamon may slow that enzyme down a bit. In theory, if apalutamide is cleared more slowly, levels could creep up and you might notice more of its usual effects, like fatigue, joint pain, rash, high blood pressure, or in rare cases seizures.

Here's the honest part: this is based only on a test-tube study, not on people, so I wouldn't lose sleep over it. Sprinkling cinnamon on your oatmeal is very unlikely to matter. The bigger question is concentrated cinnamon capsules. Before adding any cinnamon supplement alongside Erleada, please run it by your oncology pharmacist or doctor first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
