These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Padang Cassia interacts with whole categories of medication. Avacopan falls into 2 of those categories, so it can interact with Padang Cassia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 2Why this applies to Avacopan
Avacopan falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep an eye on with Avacopan (Tavneos): it can be hard on the liver on its own, and your doctor likely checks your liver enzymes while you're on it. Padang cassia is a cassia-type cinnamon that contains coumarin, a natural compound that has stressed the liver in animal studies and at very high doses in people. The worry is simply doubling up two things that can each tax the liver.
This concern is theoretical, based on how these compounds behave rather than reported cases, so I wouldn't lose sleep over the cinnamon on your toast or in your cooking. Concentrated cinnamon capsules are a different story. Before adding a daily cinnamon supplement, run it by your pharmacist or doctor so they can factor it into your liver monitoring.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Avacopan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version for Avacopan (Tavneos): your body relies on a liver enzyme called CYP3A4 to clear this medicine out. In lab tests, a concentrated extract of Padang cassia cinnamon appeared to slow that same enzyme down. If that happened in real life, Avacopan could build up a bit higher than usual, which might mean stronger effects and a greater chance of side effects like nausea, headache, dizziness, upper respiratory symptoms, or changes in your liver tests.
I'll be honest with you, though: this is based only on test-tube research, not on people, so it's more theoretical than proven. A sprinkle of cinnamon on your oatmeal is very unlikely to matter. Concentrated cinnamon capsules are the bigger question. Since Avacopan is a serious medication, please check with your pharmacist or doctor before adding any cinnamon supplement.

About Padang Cassia
What is Padang Cassia?
- Adding flavor to food
- Digestive upset
- Blood sugar support
- Antimicrobial purposes
- Antioxidant support
Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.
About Avacopan
What Is Avacopan Used For?
Brand name: Tavneos
Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.
Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
