Copanlisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for copanlisib (Aliqopa). Your body relies on a liver enzyme called CYP3A4 to help clear this cancer drug out of your system. In lab testing, an extract of Padang cassia cinnamon slowed that same enzyme down. If that held true in people, copanlisib could clear more slowly and build up, which in theory means stronger effects and more side effects, things like high blood sugar, high blood pressure, low white blood cell counts, infections, or diarrhea.

I want to be honest about the evidence: this is only a test-tube finding, not something shown in real patients, so it's theoretical. Sprinkling cinnamon on your food is very unlikely to matter. Concentrated cinnamon supplements are the real question. Since copanlisib is serious chemo, please check with your oncology team before adding any cinnamon supplement.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts.
