Cyclophosphamide falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the simple version: cassia cinnamons like Padang cassia naturally carry a compound called coumarin, and in large amounts coumarin can stress the liver. Cyclophosphamide can already be tough on the liver in some people, so the worry is that piling a concentrated cinnamon supplement on top could add to that strain.

This concern is theoretical, based on how these things behave rather than reports of the two together, so don't panic. The amount you sprinkle on oatmeal or toast is not the issue. Concentrated cinnamon capsules, taken daily, are where coumarin adds up. Since cyclophosphamide treatment already calls for careful liver monitoring, please run any cinnamon supplement past your oncology team or pharmacist first.

Cyclophosphamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's a twist worth knowing: cyclophosphamide is a prodrug, meaning it doesn't do much until your liver enzymes (including CYP3A4) convert it into its active, cancer-fighting form. So the usual rule flips. Lab research suggests Padang cassia can block CYP3A4. In theory, that could slow how much cyclophosphamide gets activated, which might mean less of the active drug doing its job. The picture is genuinely mixed, though, since several enzymes share this work.

The honest truth is this is based only on test-tube studies, not on real patients, so we can't say how much it matters in practice. Sprinkling cinnamon on your food is no concern. Concentrated cinnamon capsules during chemotherapy are a different story, so please run that by your oncologist or pharmacist first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Cyclophosphamide

What Is Cyclophosphamide Used For?

Brand names: Cytoxan, Neosar

Cyclophosphamide is used to treat many types of cancer in adults and children, including malignant lymphomas (such as Hodgkin's disease and Burkitt's lymphoma), multiple myeloma, several forms of leukemia, breast cancer, ovarian cancer, neuroblastoma, retinoblastoma, and advanced mycosis fungoides (a type of skin lymphoma). It is often given alongside other chemotherapy drugs rather than on its own.

Oral and injectable powder formulations (but not the Cyclophosphamide Injection solution concentrate) are also approved for minimal change nephrotic syndrome — a kidney condition — in children who have not responded well to, or cannot tolerate, steroid therapy. The injectable solution concentrate is approved for adult patients only due to its alcohol content, and is not approved for use in children.
