Dronedarone falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing with Dronedarone (Multaq): it can put a strain on your liver, and in rare cases it has caused serious liver problems. That matters here because Padang cassia, a type of cassia cinnamon, naturally contains coumarin, a compound that can also be hard on the liver when taken in large, concentrated amounts.

So the worry isn't that cinnamon changes how Dronedarone works in your heart. It's that two things that can stress the liver might add up. Watch for signs like yellowing skin or eyes, dark urine, belly pain on the upper right side, or unusual tiredness. The amount of cinnamon you sprinkle on food is very unlikely to be a problem. This caution is really about daily concentrated cinnamon capsules. Run those by your pharmacist or doctor first.

Dronedarone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for you. Your body relies on a liver enzyme called CYP3A4 to clear dronedarone (Multaq) out of your system. Lab studies suggest that concentrated Padang cassia cinnamon may slow that enzyme down. If that happens, dronedarone could build up a bit more than expected, which in theory might nudge up its effects and side effects, things like slower heart rate, dizziness, tiredness, or stomach upset.

I want to be honest about how soft this evidence is. It comes only from test-tube research, not from people, so the real-world risk is likely small. Sprinkling cinnamon on your oatmeal is no concern. Before starting a concentrated cinnamon supplement, though, please run it by your pharmacist or doctor, since dronedarone is a heart medicine that deserves care.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Dronedarone Used For?

Brand name: Multaq

Multaq (dronedarone) tablets are used to reduce the risk of being hospitalized for atrial fibrillation (AF) — an irregular, often rapid heart rhythm. It is approved for adults who are currently in normal sinus rhythm but have a history of paroxysmal (comes and goes) or persistent atrial fibrillation.

Multaq is not appropriate for everyone with AF. It must not be used in people with permanent atrial fibrillation or in those with recently worsened heart failure requiring hospitalization — these patients face a doubled risk of death, stroke, and hospitalization if they take it.
