Fruquintinib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the practical concern with pairing concentrated cinnamon supplements and Fruquintinib (Fruzaqla). This isn't about how your body breaks the drug down. It's simpler than that: both can be tough on your liver. Fruquintinib, a targeted cancer therapy, is already known to raise liver enzymes and can cause liver injury on its own, so your oncology team monitors your bloodwork closely while you're on it.

Cassia cinnamon contains coumarin, which in large concentrated doses has been linked to liver stress. Stacking two liver stressors could, in theory, add up. This is a theoretical caution, not something proven in people taking these two together. A sprinkle on your oatmeal is fine. Before starting any concentrated cinnamon capsule, please check with your pharmacist or oncologist first.

Fruquintinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the situation with your cancer medication Fruzaqla (fruquintinib) and cinnamon supplements. Fruquintinib is partly cleared from your body by a liver enzyme called CYP3A4. In test-tube studies, a concentrated extract of Padang cassia slowed that same enzyme down. If that were to happen in a real person, fruquintinib could build up a bit more than expected, which could mean stronger side effects like high blood pressure, mouth sores, fatigue, hand-foot skin reactions, or bleeding problems.

I want to be honest though: this is only lab data, not something proven in people, so the real-world risk is uncertain. Sprinkling cinnamon on your oatmeal is very unlikely to matter. Before starting any concentrated cinnamon supplement, please run it by your oncology team or pharmacist first.

What Is Fruquintinib Used For?

Brand name: Fruzaqla

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.

Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
