Maraviroc falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the gist for Selzentry (maraviroc): this isn't a metabolism tug-of-war where cinnamon changes your drug levels. Instead, the worry is about your liver. Maraviroc carries its own small risk of liver irritation, and concentrated cassia cinnamon supplements like Padang Cassia contain coumarin, which can also stress the liver when taken in large amounts. Stacking two things that lean on the liver is the concern.

To be clear, this is a theoretical caution based on how these compounds behave, not proven cases. The amount of cinnamon you sprinkle on food is not the issue here. High-dose cinnamon capsules are. Watch for things like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before starting any cinnamon supplement.

Maraviroc also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for your HIV medicine, maraviroc (Selzentry). Your body clears it mainly through a liver enzyme called CYP3A4. Lab tests suggest a concentrated extract of Padang cassia cinnamon can slow that enzyme down. If that happened in real life, maraviroc could build up in your blood, which might mean stronger effects and more side effects like dizziness, headache, stomach upset, or feeling lightheaded when you stand up.

I want to be honest about how soft this evidence is. It comes only from test-tube research, not from people, so we really don't know if it matters at normal doses. Sprinkling cinnamon on your food is very unlikely to cause trouble. Before starting a concentrated cinnamon supplement, though, please run it by your pharmacist or HIV doctor first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Maraviroc

What Is Maraviroc Used For?

Brand names: Celsentri, Selzentry

Maraviroc treats a specific type of HIV-1 infection called CCR5-tropic HIV-1. It is always used alongside other antiretroviral (anti-HIV) medicines — never alone. A special test called a tropism assay must be done before starting treatment to confirm you have the right type of HIV.

Selzentry (the brand-name oral solution and tablet) is approved for adults and pediatric patients weighing at least 2 kg. Generic maraviroc tablets are approved for adults and pediatric patients aged 2 years and older weighing at least 10 kg. Neither form is recommended for patients whose HIV is classified as dual/mixed-tropic or CXCR4-tropic.
