Naltrexone falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to keep in mind with this pairing. Naltrexone can be a little tough on the liver, especially at higher doses, which is why your doctor may check your liver enzymes while you're on it. Padang cassia (a cassia-type cinnamon) naturally contains coumarin, and in large or concentrated amounts coumarin has been linked to liver irritation. So the worry is simply about stacking two things that lean on the liver at the same time.

To be clear, this is a theoretical concern based on how these compounds behave, not a well-documented reaction. Sprinkling cinnamon on your oatmeal is not the issue. Concentrated cinnamon capsules taken daily are. If you're considering a supplement, run it by your pharmacist or doctor first, and report any yellowing skin, dark urine, belly pain, or unusual tiredness.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. What Is Naltrexone Used For?

Brand names: Trexan, Depade, Vivitrol

Naltrexone Hydrochloride tablets are used to treat alcohol dependence and to block the effects of opioids in people with opioid dependence. The extended-release injectable suspension — sold as Vivitrol and also available as a generic injectable — is used to treat alcohol dependence and to prevent relapse to opioid dependence after a person has completed opioid detoxification.

All formulations are meant to be part of a broader treatment plan that includes counseling and other support — naltrexone works best when it is not used alone.
