nilotinib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to know about nilotinib (Tasigna): it can be tough on the liver all on its own. That's why your care team checks your liver enzymes regularly while you're on it. Padang cassia, a cassia-type cinnamon, naturally contains a compound called coumarin, which in large or concentrated amounts has been linked to liver irritation in sensitive people.

The worry isn't a chemical reaction between the two. It's more that stacking two things that can stress the liver may add up. This concern is theoretical, based on how coumarin behaves, so don't panic. Sprinkling cinnamon on your oatmeal is fine. Concentrated cinnamon capsules are a different story. Before adding any, run it by your pharmacist or oncologist first.

nilotinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the situation with cinnamon and your nilotinib (Tasigna). Your body relies on a liver enzyme called CYP3A4 to break down nilotinib and keep its level in a safe range. Lab studies suggest a concentrated extract of Padang cassia may slow that enzyme down. If that happened in real life, nilotinib could build up higher than intended, which might mean more side effects like nausea, fatigue, headache, rash, or changes to your heart rhythm (a QT prolongation that nilotinib already carries a warning about).

Now, the important context: this is based only on test-tube research, not people, so it's theoretical. Sprinkling cinnamon on your food almost certainly won't matter. Concentrated cinnamon capsules are the real question. Since nilotinib is a serious medication, please check with your oncology pharmacist or doctor before adding any cinnamon supplement.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Nilotinib

What Is Nilotinib Used For?

Brand name: Tasigna

  • Nilotinib is used to treat certain types of chronic myeloid leukemia (CML; a type of cancer of the white blood cells) who have recently found to have this condition in adults and children 1 year of age and older. It is also used to treat certain types of CML in adults whose disease could not be treated successfully with imatinib (Gleevec) or adults who cannot take imatinib because of side effects. Nilotinib is also used to treat certain types of CML in children 1 year of age or older whose disease could not be treated successfully with other tyrosine kinase inhibitor therapies or who cannot take these medications because of side effects. Nilotinib is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps to stop or slow the spread of cancer cells.
