Nirogacestat falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to know about pairing concentrated cinnamon supplements with Nirogacestat (Ogsiveo). This isn't about cinnamon changing how the drug works or how fast your body clears it. Instead, both can be tough on the liver. Nirogacestat can raise liver enzymes on its own, and cassia cinnamons like Padang cassia contain coumarin, which in large concentrated amounts has been linked to liver stress. Stacking two things that lean on the liver could, in theory, add up.

To be fair, this is a theoretical concern based on how these compounds behave, not proven cases with this exact combo. Sprinkling cinnamon on your food is very unlikely to matter. High-dose supplement capsules are the real question. Since you're on regular liver monitoring anyway, please run any cinnamon supplement past your oncology team first.

Nirogacestat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for anyone on Ogsiveo (nirogacestat), the medicine used to treat desmoid tumors. Your body clears this drug largely through a liver enzyme called CYP3A4. In test-tube studies, an extract of Padang cassia (a cassia-type cinnamon) slowed that same enzyme down. On paper, that means nirogacestat could hang around longer and build up, which might push its effects and side effects higher. For nirogacestat that could look like worse diarrhea, nausea, mouth sores, rash, or fatigue.

I want to be honest about how thin this evidence is. It comes only from lab work, not from people, so we truly don't know if it matters in real life. Sprinkling cinnamon on your oatmeal is very unlikely to be a problem. Concentrated cinnamon capsules are a different story, so please run those by your oncology team or pharmacist first.

Brand name: Ogsiveo

Ogsiveo (nirogacestat) is used to treat adults with desmoid tumors that are growing or progressing and require systemic (whole-body) treatment. Desmoid tumors are rare, locally aggressive growths that arise from connective tissue.

This indication is specific to Ogsiveo tablets taken by mouth. It is not approved for use in children, as safety and effectiveness have not been established in pediatric patients.
