Osimertinib Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Your body relies on a liver enzyme called CYP3A4 to help clear osimertinib (Tagrisso). In a lab dish, an extract of Padang cassia slowed this enzyme down. The thinking goes that if that same effect happened in people, less of the drug would be broken down, so more could build up in your system. With osimertinib, higher levels could mean more of its usual side effects, things like diarrhea, mouth sores, dry or itchy skin, nail changes, and tiredness.

Here's the reassuring part: this is only test-tube evidence, not something proven in patients. Sprinkling cinnamon on your food is very unlikely to matter. Concentrated cinnamon capsules are a different story, so please run those by your oncology team or pharmacist before starting them.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
