Pexidartinib Hydrochloride falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the key thing to know: pexidartinib (Turalio) is already pretty hard on the liver. It carries a serious boxed warning for liver injury, and people on it get regular blood tests to watch their liver enzymes. So the concern with Padang cassia isn't about it changing how the drug is broken down. It's that this type of cinnamon contains coumarin, which in large amounts can stress the liver on its own. Stacking two liver stressors together could make problems more likely.

The evidence here is mostly theoretical, drawn from how coumarin behaves, so I don't want to alarm you. Sprinkling cinnamon on your oatmeal is not the issue. Concentrated cinnamon capsules are a different story while you're on pexidartinib. Please run any cinnamon supplement past your oncology team or pharmacist first.

Pexidartinib Hydrochloride also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the simple version: your body relies on a liver enzyme called CYP3A4 to clear pexidartinib (Turalio). Lab studies suggest that concentrated Padang cassia (a cassia cinnamon) may slow that enzyme down. If that happened in people, pexidartinib could build up and push its effects and side effects higher, things like liver problems, nausea, fatigue, hair color changes, or swelling. Since this medicine already carries a serious liver warning, that's worth taking seriously.

That said, this is only test-tube evidence, not proven in humans, and sprinkling cinnamon on your food is very unlikely to matter. Concentrated cinnamon capsules are the real concern. Please check with your oncologist or pharmacist before adding any cinnamon supplement.
