Ponatinib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the concern with this pairing. Ponatinib (Iclusig) is already known to be tough on the liver, and your care team will be watching your liver enzymes closely while you're on it. Padang cassia, which is a cassia-type cinnamon, naturally contains coumarin, a compound that can stress the liver when taken in large, concentrated amounts.

Stacking two things that can each strain the liver may add up, which could mean a higher chance of liver irritation or rising liver enzyme levels. To be clear, this worry is theoretical and based on how coumarin behaves, not on solid human studies with ponatinib. Sprinkling cinnamon on your food is very unlikely to matter. But before starting a concentrated cinnamon supplement, please check with your oncologist or pharmacist first.

Ponatinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for Ponatinib (Iclusig): your body relies on a liver enzyme called CYP3A4 to break it down and clear it. Lab research suggests concentrated Padang cassia (a cassia cinnamon) may slow that enzyme down. If that happened in a person, ponatinib could build up a bit higher than expected, which could mean stronger effects and more side effects like high blood pressure, fluid retention, headache, fatigue, or, more seriously, the blood clot and heart problems this drug already carries warnings about.

I want to be honest, though: this is only test-tube evidence, not human studies, so the real-world risk is uncertain. Sprinkling cinnamon on your oatmeal is very unlikely to matter. Concentrated cinnamon capsules are a different story. Since ponatinib is a serious cancer drug, please check with your oncologist or pharmacist before adding any cinnamon supplement.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Ponatinib

What Is Ponatinib Used For?

Brand name: Iclusig

Ponatinib, sold as Iclusig, treats two types of blood cancer in adults. For Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph+ ALL), Iclusig is used in combination with chemotherapy for newly diagnosed patients, or as a single agent for patients who cannot take other kinase inhibitors or who have a specific mutation called T315I. For chronic myeloid leukemia (CML), Iclusig treats chronic phase CML when at least two prior kinase inhibitors have not worked or caused intolerable side effects, accelerated or blast phase CML when no other kinase inhibitors are suitable, and any phase of CML with the T315I mutation.

Iclusig is not approved or recommended for newly diagnosed chronic phase CML.
