Ponesimod falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for Ponvory (ponesimod) and Padang cassia cinnamon. Your body relies partly on a liver enzyme called CYP3A4 to clear ponesimod. In test-tube studies, a concentrated extract of this cinnamon slowed that enzyme down. The theory is that if the enzyme works less, ponesimod could build up a bit and its effects, along with side effects like a slower heart rate, headache, dizziness, raised liver enzymes, or higher blood pressure, might become more noticeable.

That said, this is only lab data, not human evidence, so the real-world risk is uncertain. Sprinkling cinnamon on your food is very unlikely to matter. Concentrated cinnamon capsules are the bigger question. Before starting any cinnamon supplement, just run it by your pharmacist or doctor first.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice.
