Repotrectinib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing to keep in mind if you're on Repotrectinib (Augtyro) for lung cancer. This medication can already put stress on your liver, and your care team likely checks your liver enzymes with blood work while you're taking it. Padang cassia is a cassia-type cinnamon that naturally contains coumarin, a compound that in high amounts has been linked to liver injury.

The worry isn't about cinnamon in your oatmeal or a sprinkle in your coffee, that amount is fine. The concern is concentrated cinnamon capsules or extracts taken daily on top of a drug that's also hard on the liver. To be clear, this is theoretical and based on how coumarin behaves, not on proven cases with this exact drug. Still, since Repotrectinib's liver effects matter, please check with your oncologist or pharmacist before adding any cinnamon supplement.

Repotrectinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the quick picture with Augtyro (repotrectinib), a targeted cancer medicine for certain lung and solid tumors. Your body relies on an enzyme called CYP3A4 to break this drug down and clear it out. Padang cassia, in lab tests, appears to slow that same enzyme. In theory, a blocked enzyme means repotrectinib could linger and build up, which may push its effects and side effects higher (think more dizziness, numbness or tingling, trouble with coordination, nausea, or shortness of breath).

Now the important part: this warning comes only from test-tube research, not from people, so it's a theoretical concern. A sprinkle of cinnamon on your oatmeal is very unlikely to matter. Concentrated cinnamon capsules are a different story, so please run those by your oncologist or pharmacist first.
