Here's the thing to keep in mind with Kisqali (ribociclib): this medicine can already be tough on the liver, and your oncology team checks your liver blood tests regularly because of that. Padang cassia, a type of cassia cinnamon, naturally contains a compound called coumarin, which in large amounts has been linked to liver stress. So the worry is simply doubling up: two things leaning on the liver at the same time.
This is a theoretical concern based on how these compounds behave, not proof it will happen to you. Sprinkling cinnamon on your oatmeal is not the issue. The caution is about concentrated cinnamon supplements taken daily. Please run any cinnamon capsules or extracts past your oncologist or pharmacist before starting, and don't change your Kisqali on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Your body relies on a liver enzyme called CYP3A4 to break down ribociclib (Kisqali). Lab studies suggest that concentrated Padang cassia (a cassia-type cinnamon) may slow that enzyme down a bit. In theory, if ribociclib is cleared more slowly, more of it could build up in your system, which might mean stronger side effects. With Kisqali that can include things like low white blood cell counts, nausea, tiredness, and changes to your heart rhythm (QT prolongation), which doctors already watch closely.
To be fair, this is only test-tube evidence, so we don't really know if it matters in people. Sprinkling cinnamon on your food almost certainly won't cause trouble. The caution is more about concentrated cinnamon capsules. Since Kisqali is a serious cancer medicine, please check with your oncology team or pharmacist before adding any supplement.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. What Is Ribociclib Used For?
Brand name: Kisqali
Kisqali (ribociclib) tablets are used in adults with hormone receptor (HR)-positive, HER2-negative breast cancer. For early breast cancer (stage II or III, at high risk of coming back), Kisqali is given alongside an aromatase inhibitor as adjuvant treatment — meaning treatment after initial surgery to reduce the risk of the cancer returning.
For advanced or metastatic breast cancer (cancer that has spread), Kisqali is used in combination with either an aromatase inhibitor or fulvestrant — as the first hormonal treatment tried, or after the cancer has grown despite prior hormone therapy.
