Ripretinib falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.

There is some concern that ingesting large amounts of Padang cassia might cause hepatotoxicity in some people. Padang cassia contains coumarin. Coumarin has caused hepatotoxicity in animal models. In otherwise healthy humans, very high doses of coumarin, from 50-7000 mg daily, can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.

Here's the thing with Ripretinib (Qinlock): it can be hard on your liver on its own, and your oncology team usually keeps an eye on your liver lab values while you're on it. That's where concentrated cinnamon supplements come in. Padang cassia naturally contains coumarin, which in large amounts has been linked to liver irritation in animals and in people taking very high doses.

The concern here is mostly theoretical and based on how coumarin behaves, not on real-world reports with this drug. Still, stacking two things that can stress the liver isn't ideal. Sprinkling cinnamon on your food is fine. Concentrated cinnamon capsules are a different story, so check with your pharmacist or oncologist before taking them while on Ripretinib.

Ripretinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Here's the gist for your Qinlock (ripretinib). Your body relies on a liver enzyme called CYP3A4 to break ripretinib down and clear it out. In lab tests, an extract of Padang cassia cinnamon slowed that same enzyme down a bit. If that holds true in people, your ripretinib could clear a little more slowly and build up, which might mean stronger effects and more side effects (things like fatigue, hair loss, muscle aches, high blood pressure, or hand and foot skin reactions).

I want to be honest though: this is only a test-tube finding, not something proven in real patients, so the risk is theoretical. Sprinkling cinnamon on food is very unlikely to matter. Concentrated cinnamon capsules are the concern. Since ripretinib is serious cancer therapy, please check with your oncologist or pharmacist before adding any cinnamon supplement.
