Here's the thing to keep in mind with this combo pill: the codeine portion is the part that overlaps with passion flower. Codeine is an opioid that can make you drowsy, and passion flower is thought to gently boost GABA, the brain's calming signal. Stacked together, those calming effects may add up, so you could feel sleepier, foggier, more lightheaded, or have slower breathing than you'd expect from the medicine alone.

This is a theoretical concern based mainly on how these two work, not strong human studies, so I don't want to alarm you. Still, until you've talked it over with your pharmacist or doctor, go easy, especially before driving or having a drink, since alcohol piles on the same sleepiness.

In vitro research suggests that passion flower can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the bigger picture with this combination pill (acetaminophen, caffeine, and codeine). Some early lab research hints that passion flower might gently speed up a liver enzyme called CYP3A4, which helps process certain medicines. On paper, that could slightly lower the levels of some of these ingredients in your body.

The good news is that this is very weak, theoretical evidence (test-tube studies only), and the effect seen was far smaller than known strong inducers. So for everyday use, I wouldn't expect a meaningful drop in pain relief. The more practical concern is overlap in sedation: codeine can make you drowsy, and passion flower is calming too, so together you may feel sleepier or foggier. If you're using both, just mention it to your pharmacist or doctor.
